Credit Terms & Agreement

In support of this application, Itec 2000 Equipment Inc. is hereby authorized to obtain credit and/or financial information from my/our bank(s), other financial institutions or commercial firms with whom I/we have done business. It is understood that any such credit and/or financial information will be held in the strictest confidence and used only in consideration of this application.

The undersigned hereby agrees that the terms of sale are Net 30 days. The amount becomes past due if not paid by the end of the month, and a 2% services charges (26.8% per annum) will be added on any past due portion. Should the undersigned not pay Itec 2000 Equipment Inc according with the terms, it is understood that credit privileges may be withdrawn. In the event of default in payment and if the same is placed in the hands of an attorney for collection, the undersigned agrees to pay all costs of collection, including a reasonable attorney's fee.
